Perioperative Incident Database.
The purpose of this Database is to collect data on events which may have harmed our patients or may have come close to doing so. With time, we may be able to see patterns in our perioperative care which can be improved or corrected. A simple example might be that two very similarly appearing medication vials should not be kept side-by-side in a medication drawer.
This WebSite has been published by the Department of Anesthesiology of The Ottawa Hospital for the use of any clinical staff who has knowledge of an incident relating to the perioperative care of a patient.
We invite not just anesthesiologists (both staff and resident) to participate but also surgeons (staff or resident), anesthesia technicians, anesthesia nurses, OR, OBS and PACU nurses, ward nurses, clinic nurses, OR attendants (PCA's), and anyone else who thinks that an event has happened that deserves reporting.
Please note: Reporting an event to this database does not replace the official incident reporting mechanism for The Ottawa Hospital.
If you choose to report only to this database, you may choose to remain anonymous. However, we encourage you to enter your name and contact method so we can reply to your report. This is not a process to lay blame but to improve patient care and safety.
